REQUIREMENTS:

• Complete Form & Application S-1
• 2 Current Photographs of the child
• Original birth certificate with translation
• Passport Copy of parents & proof of Pakistani nationality.
• Marriage certificate.
• Letter of Consent from Parent (if one of them is a foreigner).
